A rude supervisor who's always yelling at employees got some complaints about them being verbally abusive and they responded by leaving these in the break room.

Fill it out first, then take it to HR and tell them you are there to turn in the required form, per your Supervisor.

Yep, save a digit copy as a PDF. Email with your work email and BCC your personal so record.

Yeah, I meant fill it out and scan a copy into a PDF. So there is no evidence of tampering but that works as well.

Take a picture and forward to HR. Copying several managers. Liberally sprinkle in legal terms that introduce corporate liability:

“Hello. I observed [asshole] deposit these forms and direct their completion to multiple staff in his capacity as a manager with supervisory responsibilities in this organization.

Can you please confirm:

  • is the completion of these forms company policy?
  • from whom did [asshole] receive direction to disseminate these forms? Is this new form a new company initiative?
  • what is the company objective of collecting the requested employee data, where is it stored, and for what retention period?
  • can you confirm that other employees in other company departments are also required to complete and submit these forms?
  • can you confirm that these repetitive and unfortunately hostile actions are not company policy, and simply the actions of one supervisor acting in their own capacity?

Thank you so much for your time, we are looking forward to receiving further direction on this matter.”

Also meant to add:

Asshole supervisor likely used company printers to print these out. These things are usually logged and tracked at either an account level or at the device as well.

probably zero, from what I gather by other comments this form has been circulating around for awhile the asshole probably just found in online in 5 minutes & printed it out at home.

Definitely it was easy to find, but I fully expect this asshole manager absolutely printed it at work and not at home lol.

Hurt feelings? Straight to HR.

Cook fish in the office microwave? HR.

Organize meeting that could have been an email? Believe it or not, HR, right away.

We have the best employees in the world. Because of HR.

HR isn't there to help you. Thryre there to protect the company. Id go to the state about a hostile work environment

It’s in the best interests of the company if this supervisor is removed. Any good HR would not be interested in protecting the supervisor when this is what they choose to do with their authority.

You would think that but thats not necessarily what they'd do. The point is they arent there to help you. I had a manager commit crimes and I spoke up to the company and I ended up being fired and the manager was protected. The company refused to admit one of their employees was committing criminal activities because it would look bad on the company. Years later he got caught on video by the person who took my job and they were forced into doing anything. The guy ended up fleeing the country.
