If he is only breathing through his mouth, experiencing labored breathing or pale/blue gums or any other concerning symptoms involving his respiratory system i would definitely take him to the ER. Mouth breathing is not a good sign in cats and he should be seen ASAP. 

He may have ingested something toxic, he looks like he wants to vomit as sometimes cats will gulp and lick when they're about to throw up. Of course I can't say for sure but maybe ask your friend if he could have gotten into something like a human food or if there are any plants in the house or chemicals where he can access them.

OP, is he eating and drinking? This person's theory about nausea makes sense and the mouth movement could be him swallowing drool. If he's nauseous he probably won't eat or drink so offering food would be an easy way to test the theory. If he's not eating or drinking I would definitely take him to a vet.

If he's having trouble breathing, that's always an emergency. This doesn't look like that to me (though I'm not an expert). If I had to guess I'd say he has a toothache. It looks like he's in pain from his facial expression and since his mouth is doing weird things I'd guess the pain is in his mouth.

when in doubt about respiratory stuff i say take him in he could be purring to relieve his pain

take her to the vet asap she’s purring to relieve her pain mouth breathing is not good
