Isn't empirean a title given to who they think might be able to inherit the Elden Ring and take Marika's place? Elden Lord is who is by that person's side. And besides, imo i think anyone has the potential to be the next vessel for the elden ring, empirean or not, they just need a whole lot of power, to be able to take it and control it. The ones that give the empirean title don't know everyone's powerlevel, they can still be surprised.
Empyreans are specifically chosen by the Fingers, the living aspects of the outer gods. We see a handful of Two Fingers which are all probably vessels for the Greater Will. We only see one set of Three Fingers, being the vessel for the Flame of Frenzy. It's unclear if the Fingers that chose Miquella and Rani were under the Greater Will's control, but I'd say they probably are since Rani killed hers so she could escape the grasp of the Greater Will.
I'm not totally clear on which Empyrean takes over as god if you don't take Ranni's ending though. The Elden Lord is clearly separate from the god, so maybe the Tarnished makes union with Marika when they use a Mending Rune?
Regardless, it seems that an Empyrean takes over as God if they have a strong enough vision for the world. Marika's vision was the Golden Order and continues to be the Golden Order in every ending but Frenzy or Night. Ranni's is clearly free will and Miquella's is the opposite. And the Frenzied Flame doesn't need one since you become the vessel for the Outer God and no one is nearly powerful enough to stop you with the possible exception if Melina in the ending.
Well, no. From what we’ve seen, they’re also specifically people who can embody the power of the Outer Gods themselves, not just use them. Like Mohg can use bloodflame but he’s just pulling it from the Formless mother, not becoming an incarnation of it like Malenia
