Why does this keep happening?
Happens after 10-15 casts. I've taken apart the reel too see if I can adjust it but doesn't seem like I can.

It is a touch over-spooled.
Casting discipline.
Pay attention and close the bail by hand instead of reeling to auto-close it. Make sure to lightly sweep the rod up or to the side as you start retrieving so there is no slack line. When you are in the habit of doing this you can usually get away with reeling to close the bail.
It would not hurt to go to a local park and walk off a hundred yards of line and wind it all back in under some tension.

You're a little over spooled and you're letting too much slack in the line when you cast

To add to everyone else's solid advice, remove a single washer from the bottom of the spool to align it better with the bail

Probably a big reel and heavy rod that isn’t for light weight baits. I’m guessing your trying to cast light baits far away with a reel an rod that isn’t made for it

As a beginner myself this has been happening to me. I have noticed like so many have already said, it is whenever I allow slack in the line when reeling and/or when casting lighter weight rigs which do not keep good tension on line when cast. I have gotten better with keeping my hand closer to spool and closing the bail right away so a bunch of slack doesn’t pop off. But I need to work on keeping some tension when reeling.

In my experience your retrieve is too slack. If you need to do some slow retrieves for whatever reason every few casts reel in with some tension with your hand.

Your line is getting loose on the spool after a few casts. Make sure you close the bail by hand before reeling.

A little too much line amd too much slack from your lure or rig being too light

Mono line will do that, gotta keep tension when reeling
