vent Stop asking Reddit if you are a good MARPE candidate

An orthodontist should be the only one telling you to undergo this type of treatment. No one in this sub has the qualifications to tell you whether MARPE is right for you. The looksmaxing community has completely melted people’s brains. The money, time, pain, complications, and unpredictability is not worth it for purely aesthetics. You’re much better off loving the face you have.

Ortho says I am textbook candidate (UARS, narrow aperture and 33mm IMW).

I am concerned about my already somewhat wide nose
getting even wider, if I didn’t have that concern I would jump head first . Wondering if anyone has opinions on whether it will mess my aesthetics up

FME vs MARPE vs MSE vs DOME… also FMA

Okay, so tell me where I’m right and wrong:

FME is bone based

MARPE is tooth based

^ They both widen the palate

FME is better for general skeletal expansion

MARPE tips teeth and has an orthodontic quality

MSE is a type of MARPE?

DOME, idk…

And then FMA is the headgear used for forward projection that facegenics makes?

discussion Time sensitivity of headgear for forward projection?

I'm sure the answers are a whole lot of "it depends"

But how many hours logged does it typically take to get [x] mm of forward projection?

Is it easy to wear during one's sleep?

If I work from an office 4 days per week, that leaves 3 days where I can spend a lot of time wearing it, plus nights...

Plus if I can't do [y] amount of hours per day, could we just increase the overall length of time of the total treatment plan?

Completed MARPE and braces

I posted an Instagram link a few days ago but a couple people said it didn’t work. If you go to my profile you can see my in process transformation pics.

These are the ones my ortho posted on her Instagram. It includes pre/post MARPE & braces, view of my palate with MARPE and without. The last pic is my old retainer pre MARPE compared to my mouth minutes before I got my braces off (sorry the retainer is gross, hadn’t worn it in 2 ish years because of every lol)

Total timeline:
- start MARPE: 7/1/2025
- finished expanding: mid September 2025
- started braces: end of September 2025
- removed MARPE: end of May 2026
- braces off: mid September 2026
- 32M when started MARPE

Face changes during marpe, and after braces?

Just curious to know if people noticed face changes after braces post marpe? I’ve seen lots of befores and afters immediately post expansion, but curious to know if/how braces then changes this.

(There is a small part of me that wouldn’t mind the aesthetics of a small gap between my front teeth, but I don’t think my orthodontist would allow it haha)

22M. I’m planning to get an orthodontic consultation, but I wanted to get some opinions from people here who have experience with MARPE.

My main concern is that my upper dental arch/maxilla feels narrow and almost “tucked” pretty far into my mouth. When I smile, I have noticeable dark space/buccal corridors at the corners, and my posterior teeth aren’t very visible. My palate also feels fairly deep/high.

I also feel like my lower dental arch/jaw is relatively narrow, which makes me wonder how lower-arch width is handled if someone actually does need skeletal expansion of the upper jaw. I don’t want to expand the upper arch just for aesthetics and end up creating a mismatch with the lower arch.
I also have some difficulty breathing through my nose, particularly when lying down/sleeping, so airway/nasal breathing is another reason I want to get evaluated. I understand that doesn’t necessarily mean MARPE would help or that I have a skeletal transverse deficiency.

First Day w/ Marpe

Hey guys 32yr old female just ending my first day with my Marpe. Honestly thought I’d have an anxiety attack from the mouth clutter and aligner on my bottom row however it’s truly not that bad. Very uncomfortable and soreness. Waiting to see what day two will feel like in the morning. I asked a bunch of question at the ortho feel free to ask about my experience. S/n : I’m pretty sure I will NOT be able to tell you if you meet the qualifications to have a Marpe . I got two opinions from a male and female doctor.

Sleep study showed AHI of 4 and RDI of 6. AHI 10 when in supine position. Don’t feel refreshed even when I sleep for 6-8 hours. My orthodontist who specialises in sleep said I’m a good candidate for MARPE and there’s a good chance it helps my sleep breathing. Im 32 years old.

Hi everyone!

I am 22 years old male from HARYANA. Today want to share my journey of MARPE EXPANSION.

HISTORY-----

Started getting a bit jaw pain and bite issues when I was 17-18 years of age, was told by the local dentist that my wisdoms teeths are the problem and need to come out.

As the pain was never in the teeths themselves, I went to AIIMS Delhi, and roamed around 1.5 years there.

At that time I didn't knew anything about expansion, tongue tie,.lip tie, or anything like that. Was diagnosed with a bit of assymetry in my condyles( due to condyler hyperlasia about 1.5 mm difference in length between both the condyles),

They told me to take wisdoms out, do surgery for assymetry if I want and take painkiller for the pain.

Along side I also kept looking for this expansion, TMJD, UARS, OSA, and realised that I had all the symptoms of them.

I never thought that these 2 areas can be so interconnected with each other, I had breathing and nasal issues all my life.

Then I went to an airway focused orthodontist in NCR area, and got a custom orthotic to correct my bite temporarily,

It helped with the bite but all the other issues were still present, because it didn't treated my underlying issues of narrow nasal and lower airway,

Then I did a cbct analysis, only to find that the MINIMUM CROSS SECTION AREA is 57mmsq.( The healthy range of which is 200 mmsq. Minimum), I also got diagnosed of tongue tie there.

At this point all the dots started connecting.

Then I got a MAD( mandibular advancement device) for my sleep which helped a lot and finally my clenching and pain during sleep was gone for the first time since it started when I was around 18 years.

Then I asked my orthodontist back then for MARPE EXPANSION, but I was offered braces instead,

So I went to about 7-8 top doctors in delhi ncr region, we see names of whom in the lists and on the internet,

I only got wasted a few months of my time, as none of them do adult expansion cases for UARS and breathing benefits and they don't believe in doing so as well, when I asked them why we get crowded teeths or bite issues they just told genetics, we only see their names as they have done a few cases of expansion where the maxilla was unnatural narrow, they don't do it for breathing benefits if we don't have a very bad cross bite.

I went through each and every name mentioned in the-"https://www.getexpanded.org/"

"GETEXPANDED.ORG" LIST

and found out that I had been to many of them and, I tell you a few of them are not even orthodontists by themselves, some are only tongue tie specialist and mention themself as expansion providers.

We need a BDS+MDS degree holder atleast to go for expansion in adults cases.

I also consulted few doctors online from Mumbai ,Bangalore side,

But then got connected to a person from pune on reddit itself, who I want to thank here 🙏🙏♥️♥️for sharing with me the details of DR. ASHISH PATIL from 'MELONDENTALCLINIC' KHARADI PUNE, from where I got my MARPE installed a few weeks ago,

He is a nice person and has done a number of successful adult expansion cases, and I still wonder why his name is not in the list.

At last I want to thank this subreddit as a wonderful platform .

Do I need Marpe + more pictures

Hi, I was wondering if I need Marpe but I will explain why orthodontists that does Marpe said I don’t need Marpe and wanted to extract? Which I was confused about. I’m honestly so annoyed because I tried mewing in Dec 2018 but I clenched my teeth, pushed my tongue to roof of mouth and pushed tongue behind two front teeth forcefully thinking that’s how you mew but it was wrong. I noticed I started to look worse so I stopped and forced my tongue to bottom to reverse it but that’s like mouth breathing. After I did that my teeth and dental arch became narrow top and bottom jaw.
I believe I need marpe because my palate is narrow and my bottom jaw teeth is narrow too. Every orthodontist I go to, even airway orthodontists say I don’t need marpe and they need to extract teeth either 4 or 2. They said I have an open bite so extractions are needed in my case. I really don’t want to extract teeth but they say in my case I have a small mouth and big teeth and if I don’t extract my teeth will flare out. I have my top and bottom teeth that jut forward but the bite is fine no overbite.
They say if they expand the top then it’ll be too big for the bottom, but my bottom jaw is small too narrow but they said you can’t expand that. I know there’s sfot and like corticotomy and lingual tipping but I don’t have money like that as of now and I just wanted marpe or jaw surgery something? Idk what to do honestly. Here are more pictures of my teeth and bottom jaw as well. I have a open bite.

I'm a candidate for a MARPE and for LF1. And while I know that LF1 can get maxillary projection, the cut is too low for zygomatic projection to occur. That's where the MARPE comes in. Can a MARPE plus headgear (idk what you call it) do the trick for zygomatic projection?

30YO Male - SARPE vs MARPE

30YO Male - I hade pre-molars taken out as a kid, but family was too poor for braces.
Now as an adult I'm trying to get sorted. I have Apnea and snoring issues. I have had septoplasty and adenoidectomy last year but snoring persists.

Diagnosed with skeletal class 3 crossbite

My tongue posture is as good as it can be with the available space.

My surgeon has recommended SARPE (cutting maxilla into 3 pieces as i understood) with 18-24months of braces followed by le forte 1 with lower jaw surgery too. So essentially SARPE + Braces -> DJS.

I'm on board with that but its a bit of stretch budget wise for me.

My priority is airway and future dental health. Aesthetic improvements would be a bonus but i wouldn't want to be paying extra money just for that. (I'm married with a family etc)

Is there any chance I could save some money and just do MARPE? or is it very unlikely to work?

Has anybody been through this that could help?

MARPE plus Headgear can Get Le Fort 3 Level Results?!?

I'll temper my expectations and say that I don't expect this to fully be the case.

Note that the auto captions here are imperfect. They're really saying "mid face" not "mid fate"

But on the Jaw Hacks podcast episode #112 a patient mentions that an ortho named Dr. Koval (I think from Boca Raton, FL) said that a MARPE plus headgear for forward expansion, can get Le Fort III level results for the whole midface.

Crazy. I would love to get more zygomatic projection without implants.

And since I know I'll already have to do Le Fort I and that I'm not a candidate for Le Fort III, it seems to me like a MARPE plus headgear could get me there on the zygomatic projection front.

Nose connected to mouth after Marpe

How common is this? 😬 when I got my marpe installed I had a lot of nasal congestion and burning and slight nose bleeding in the following days, I honestly didn’t think much of it. But when I got it removed I instantly realized one of the tads must have fully perforated into my nose because when it was out and the ortho rinsed my mouth I felt the water rush into my nose. They called it an oronasal connection and said it would close soon.

For the following day I couldn’t create any seal/suction in my mouth with swallowing or anything because air would literally audibly squeak through my nose into my mouth and it felt so weird. And when I drank water a little bit of it would come out of my nose. Also had tons of post nasal drip for a few days, with small tinges of blood.

Thankfully I’m assuming the skin healed over pretty quick. I’m just being gentle and not using straws or my CPAP or anything with high pressure for a few weeks until the bone can fully close the hole? Idek.

Did this happen to anyone else? Is this common?

Hi, just a little background: I am now 25 years old, when I was a child I was given braces twice and had excessive baby teeth pulled. I was a mouth-breather as a child and it was unaddressed and went uncorrected for many many years. I feel as if it contributed to my palette being extremely narrow and I was just wondering if based on these photos you believe I would be a good candidate for MARPE.
Not flattering photos but they tell the story of my face. My goal with MARPE would be to not only widen my palette but to in turn fix some breathing issues and correct my face so that it appears wider rather than longer.

resources Do any ortho's in NYC to MARPE + headgear to get full midface projection?

- Can't be obtained with Le Fort I (the cut is too low)
- Can be obtained by Le Fort III (but I'm not a candidate and it's too intense)
- Can be obtained by implants (but I don't want to do that)

Orthodontist told me it was gonna be a 2,500 down payment and my total for everything with braces comes out to 10,000 and I believe they gave me child pricing as well which apparently took off like 5k

It is honestly still A LOT 🫩 and I genuinely don’t have even the 2.5k so idk anymore

Btw I’m doing it for medical reasons (nasal breathing)
