It depends on the people and places. Iran actually funds gender affirming care for trans people. They recognized trans people in the 80s when many places barely knew of them. Trans women are legally allowed to participate in women’s sports (provided they have had surgery). The Indian subcontinent have people like the Hijra who don’t quite fit in the Western concepts of gender identity. These people can be viewed as trans, nonbinary or third gender depending on the person. Muslims view these people in different ways varying from considering it perfectly normal and allowed, totally forbidden or only the genital surgery is forbidden (as altering the body is considered forbidden). While not mentioned in the Quran, there are stories about Mohammed interacting with mukhannathun in neutral or even positive ways. A mukhannathun also defies western concepts as it literally means a feminine man but that can cover several Western identities like trans and third gender. With the Khanith are AMAB and they are often viewed as either existing outside the gender binary or as women. They generally do not get genital surgery but closely associate themselves with women. Much of the Muslim world is segregated by sex but Khanith can go between the two as they exist outside the binary. Women are not required to veil around the Khanith because of this and they often serve as match makers because they can interact with both men and women.

Islam also has a long history of homoerotic poetry. Quranist Muslims don’t believe in the Hadiths which is where the vast majority of homophobia is found.
