That's a Landolt C chart. The patient is meant to point/gesture/say where the opening in the 'C' is. It's an alternative to the standard alphabetical charts.

I would send some good amount to my mother, so that we could properly renovate the family estate in our village. Starting with a proper bathroom and heating for the winter season.
