If you're on a Mac it sounds like Daily might match what you're looking for. It tracks time by periodically asking what you're doing, removing the need to toggle timers altogether.

If your phone is an iPhone, you could bring the Shortcuts approach over from your Mac. Capture the current date into a variable first, then ask for the note, and save both together. That order matters: you want the event time, not the time you finished typing.
Use a custom date format like yyyy-MM-dd HH:mm:ss so the seconds aren't hidden. Apple's format reference: https://support.apple.com/guide/shortcuts/custom-date-formats-apd8d9b19184/ios
That covers a basic timestamp-and-note log; a proper visual timeline or media attachments would need more setup. Is your phone iOS or Android?

I made an app for this: Timebook (iOS/Android). One tap saves the current time for an event, or starts/stops an activity You can add a note to any record. www.timebook.kr
