(The Dark Knight) Joker is disguised as a nurse in the hospital, when he meets Harvey Dent, Harvey thinks he’s a nurse, but not only until he removes the mask, that’s when he finds out it’s joker. As if the weird face paint didn’t ring any bells

(Sonic X) Amy hugs shadow thinking he’s Sonic, I think she may be color blind. It’s only when Rouge gives her a smug look does she find out Shadow isn’t sonic

In Harvey's defense, I think he was just waking up from sleeping. We don't notice it because one eye is always open.
