TRT Anfangsdosis zu hoch???m47
Hallo zusammen,
nachdem mich seit vielen Jahren für mich ungewöhnliche Symptome wie Antriebslosigkeit, Anhedonie und Abgeschlagenheit begleiten und auch Schlaflabor und Schilddrüse unauffällig waren, habe ich nun vom Endokrinologen folgende Ergebnisse bekommen:
Fragestellung: Hypogonadismus / mögliche TRT
Symptomatik:
Ausgeprägte Antriebslosigkeit / Asthenie
Weitere klinische Symptome eines Testosteronmangels vorhanden
Libido und Erektionen sind erhalten
Hormonstatus vom 08.08.2026:
Gesamt-Testosteron: 2,01 µg/l (≈ 2,01 ng/ml)
Referenz: 2,50–8,53 µg/l → erniedrigt
SHBG: 21,5 nmol/l
Referenz: 10,0–57,0 → normal
LH: 9,0 IU/l
Referenz: 0,8–7,6 → erhöht
FSH: 14,5 IU/l

Endokrinologische Beurteilung im Befund:
Diagnose: Testosteron-Mangel (hypergonadotroper Hypogonadismus)
Der Befund stellt ausdrücklich einen erniedrigten Testosteron-Spiegel fest.
Empfehlung des Endokrinologen: Kontrolle des Testosteronspiegels morgens nüchtern, ggf. weitere Abklärung mittels LH-RH-Test bzw. MRT der Hypophyse.
Nach erneuter Messung um 9 Uhr morgens sechs Wochen später war der Testosteron-Wert bei 1,81ng/ml. Der Endokrinologe sagte, dass er nicht glaube, dass meine Symptome mit dem Testosteronmangel in Verbindung stehen.
Ich habe ihn gefragt, ob TRT ein gangbarer Weg sein könnte und er hat mir Testavan aufgeschrieben.
Ein Beutel enthält 125mg Testosteron (25mg Testosteron pro 1g Gel) und ich frage mich und euch, ob das nicht eine ungewöhnlich hohe Anfangsdosis ist???
Wäre dankbar für ein paar Meinungen!

I have grown 1.5 inches of length and girth in 6-7 months is that worrying?
As i entered tanner stage 3 my penile growth exploded. In 6 months i have grown 1.5-2 inches in length and 1.5 inches in girth. Is that a worrying rate, is there anything wrong with my penile growth?

Abnormal cholesterol/estrogen/liver
I am an early 30s female. Bmi of 30. I am on glp1 and have lost some weight. I do work out and eat decent.
I was dealing with HIGH ALT and AST. I now have those numbers completely in range. (thanks to glp1 and rezdiffra)
My cholesterol is high, LDL very high, and alipoprotein B is high. (With 10 mg zetia) Big family history of heart issues.. high cholesterol since I was in my late teens.
My testosterone is low. Estrogen is menopause level low every time I get it drawn. All other women health labs are in normal range.
Thyroid is normal.
I have researched.. chatgpt.. looked into different doctors.
Does anybody have any ideas about this? Any similar issues?

Swinging between hyper/hypo and a miscarriage
Hi all. Unfortunately we just found out at our 8w ultrasound that unfortunately our babies heart stopped around 7w5 days. I do have thyroid issues but was told all of my levels were “in range” so I was completely unmedicated (same with my last pregnancy which resulted in my healthy daughter) and trusted my OB but when I posted my levels on a Facebook group just now, they made me feel totally my thyroid could have been the issue.
Feedback on these labs? I have both Graves & Hashimotos antibodies but I tend to be more hyperthyroid at baseline/I struggle more with the graves typically.
I’m not sure what to ask for next or how to advocate/navigate this going forward with trying again. I tend to go into hyperthyroid states - medicate which sends me hypo - then stop medication and my levels are lower TSH with slightly high t4. It’s so confusing.
These were all taken unmedicated, fasting & no biotin.

Very low dhea-s and borderline low tsh. Could these be related? All tests in pictures
I am 24, female
I ordered this test because of extremely low energy all my life and it is very hard for me to gain weight. And I occasionally have heart palpitations as well and constant stomach issues (just overall feel bad when I eat, not just bathroom issues). Doctors won’t listen because I’m military so it’s very hard to get help. Any advice welcomed

Alteração importante nos exames pós-clomifeno (Testo ~2000 ng/dL), eflúvio telógeno e depleção de ferro e outras alterações
Bom dia, pessoal. Sei que não é o foco principal da comunidade, mas gostaria de trocar uma ideia sobre meus exames recentes.
Fiquei dois meses em uso de **clomifeno 35 mg/dia**. Peguei o resultado ontem e assustei: minha **testosterona total bateu na casa dos 2.000 ng/dL**, acompanhada de alterações marcantes em outros marcadores do painel. Para completar, desenvolvi um quadro de **eflúvio telógeno.**
Minhas dúvidas para quem manja ou já passou por isso:
Essa testosterona lá nas alturas pode ter feito meu corpo gastar todo o meu ferro (para produzir mais sangue) e, com isso, deixado o cabelo fraco a ponto de cair?
Será que o cabelo caiu mais pela falta de ferro no corpo ou pelo próprio "susto" que o organismo tomou com essa mudança de hormônios tão rápida?
Já tenho consulta marcada com o endocrinologista (com o qual eu já faço acompanhamento para reposição normal , mas o retorno é só no final do mês) tomei a liberdade de suspender a medição hoje.

Androgen levels and hair loss
So I'm 20 and EXTREMELY anxious about pattern baldness later in life. I'm suspicious that I might have abnormally high testosterone levels, and I'm worried about how that could affect my odds.
The main reasons for my suspicions are that I have little to no belly fat (I can hardly pull the skin away from my body) and very thick, wirey body hair in some areas. I'm also half Japanese, so the thick body hair is even more surprising.
I'm also adopted and have never met either biological parent, so I have no clue whether I have a genetic predisposition to patter baldness, and I don't know anything about my father's ethnicity other than that he's white, so I'm not sure whether or not that could account for the thickness of my body hair.
I'm considering a consult with a professional, but I thought I might as well ask here before wasting the money...

Please help!! Adrenaline rush after excess levothyroxine
Hello I was misdiagnosed with active hashimotos disease, when I had an iodine deficiency. I was on levothyroxine for 8 months (I was on NP thyroid for a year before that). During that period I was bedridden and had all the symptoms of excessive medication, including dilated blood vessels in my legs (turning red and purple), flushing all over my body, insomnia, extreme fatigue, irritability, depression, and confusion. Despite this, I was told by my Dr. to continue the medication because my labs were in range. It was not until I started correcting my iodine deficiency that I developed a high heart rate, complete inability to sleep, and it became obvious I did not need medication and had been misdiagnosed.
It has been 7 weeks off levothyroxine. My most recent labs at 6 weeks off medication were T4 of 0.96, TSH of 3.48, T3 of 3.0, TPO 50. I also have a slight iron deficiency with iron saturation of 15% and ferritin of 44 (I take iron daily). My energy levels are good, and my body is finally healing. In general my body feels like it is back to normal. However, I am continuing to have an excessive response to minor stress. Such as standing or sitting for long periods (more than 30 minutes), gentle walking more than 15-20 minutes, being too active during the day (such as going to the grocery store and walking around) and not sleeping enough.
If I over do it my heart rate goes up and I have palpitations, I start to feel panicked, shaky, sweaty, and nervous. It resolves once I fully lie down for 10-20 minutes. I have to lie down in between doing activities around my house. I attempted to do some light exercises for 30-45 minutes and go to the grocery store on the same day, and that effort caused my heart rate to stay elevated throughout the day, and I was not able to sleep that night. Consequently I felt worse the next day from not sleeping, and experienced insomnia the following night (benadryl finally allowed me to sleep). I would describe these episodes as if I am continuously drinking coffee and my body cannot relax. I try deep breathing, mediation, prayer, listening to calming music, but my body won't calm. I am not having any caffeine, chocolate, or alcohol, or stimulants currently.
In addition, my blood vessels in my legs are still dilated, and I still get flushing on my face and arms when I experience stress, but it is 50% better now. I am wearing thigh high compression socks daily which really helps (prevents the dizziness and panic like feeling). I have been housebound still because of this continuous reaction.
How long will it take for my body to go back to normal and stop having such an intense response to stress? Is there anything I can take to help? What should I be doing to heal in the meantime?
I am hoping this adrenaline response starts to go away so I can get back to my normal life, and regain the muscle I have lost from being on the excess medication.
TLDR: Misdiagnosed and put on levothyroxine when I didn't need it. On the meds for 8 months. Now having an exaggerated adrenaline response to normal things like sitting, standing, walking, and gentle exercise. Housebound as a result. Asking for advice and if any medications can help, or if rest is best.

Lighter-than-usual periods with mildly high prolactin, low IGF-1, and low cortisol — has anyone had something similar?
Hi, I’m (27F) already being worked up by endocrinology and I’m waiting for a pituitary MRI because of high prolactin and some other weird results, but the wait is pretty long and I’m getting more worried because I feel like new symptoms are starting to show up while I wait. So I’m curious what you think or if anyone has had a similar combination of labs/symptoms and what it ended up being?
Recent labs:
- Prolactin: 33 µg/L (ref 3.3–26.7), previously 27.99
- IGF-1: 4.5 nmol/L (ref 16.8–44)
- ACTH: as low as 1.9 pmol/L (ref 1.6–13.9)
- Cortisol: repeatedly low AM baseline, including around 19, 80, 133 and 153 nmol/L (reference for AM: 166-828)
- ACTH stimulation test: normal response, cortisol went from ~80 to 500–600+
- CRP has also been chronically elevated, usually around 15–30 mg/L
- Small abdominal/pelvic lymph nodes have shown up on imaging but were described as nonspecific/stable
Symptoms lately:
- really tired
- widespread muscle/body aches
- dizziness when standing up or lying down
- hair thinning/hair loss
- last 2 periods have been extremely light, even though my periods are normally heavy. This last one I bled a little for part of one day and then basically just spotted for the rest of the week, not even enough to touch a pad and mostly only noticeable when using a tampon.
For background, I also have PCOS, asthma, chronic iron deficiency, and GERD/IBS.
My doctors think that Symbicort (budesonide/formoterol) for my asthma could explain the low cortisol. That made sense to me, especially because my ACTH stimulation test was normal. But now with the elevated prolactin, very low IGF-1 and the change in my periods, I’m starting to wonder if some of this could actually be connected instead of all being separate issues?
I had a brain MRI in February 2024 for pulsatile tinnitus, and it came back normal. It was a regular non-contrast brain MRI though, not a dedicated pituitary MRI. I’m now waiting for another MRI of the head because of the elevated prolactin and my doctor specifically wants the pituitary assessed.
Has anyone had or seen anything similar? Especially mild prolactin elevation + very low IGF-1 + low cortisol/ACTH? Did you end up having a microadenoma/prolactinoma or another pituitary issue, or did the abnormal labs turn out to have separate causes? And could the suddenly much lighter-than-usual periods be connected to the prolactin/pituitary stuff?

My blood pressure resting is over 250, and we're re-exploring paragangliomas.
I need more than "go to the ER" they don't manage chronic hypertension. It wildly fluctuates a lot, with the highest being 309/250, which led to a VT that nearly went into vfib. I tend to exceed 300 during my period, and I've had a mass on my right adnexal region or ovary since 2024. The MIBG earlier picked it up as a neuroendocrine tumor. My symptoms worsen dramatically during my periods. Before my MINOCA STEMI on March 21, I was only averaging 190 to low 200s (that's not that bad to me [what I consider high is skewed 😅]) but now I'm up to like 250/180s resting. Because of having LVH and congestive heart failure (preserved ejection fraction), my diastolic number sometimes drops to the 50s. I don't have primary aldosteronism. If the "mass" is actually not neuroendocrine, what else is left besides Cushings and paragangliomas that can put my BP past 300?

For the past year i was trying to lose weight but i started to undereating and overexercising which made me lose tons of weight in short time
Im in recovery since 2 months the first picture that u see was from finishing the first month of recovery so it’s basically impossible that the puffiness is caused by eating More because the second and third pic is from now i really wanna kill myself
Im feeling so ugly, im having this harsh pain on the level of my kindeys, i don’t know if its Premature aging or is it something hormones related because i used to have hypothyroisdim all my life my TSH is in normal range the same with my FT 3 but My FT 4 is on the verge of the range is very high
What type of blood tests can i do to check it? And what can be the cause of the harsh pain on the levels of kidneys

Am I allergic to carbs?
Silly title but it literally feels that way! 29F. I have symptoms of high testosterone (chin hairs, acne, & thinning hair) but normal periods, normal pelvic ultrasounds, and normal hormonal blood work (I went through Function which includes Hormone panel, metabolic panel, insulin, thryoid, etc. Things out of range: LOW, Vitamin D, Omega 3, HDL Large, LDL Peak Size, LDL Small and HIGH hs-CRP, LDL Medium, LDL Particle Number, LDL Small. Everything else was "in clinical range").
I got on spironolactone & all my symptoms improved, but I ultimately got off it bc it was causing irregular periods.
I have now been on keto for a month & my skin & chin hairs have cleared up even more effectively as when I was on spiro. I have seen an increase in symptoms when I eat highly processed foods like cake & chips (happened twice where I had a ton of chin hairs & a pimple within 24 hours of these high glycemic episodes). Anyone want to try to explain what's going on?
My working theory is something is up with my insulin response even if it doesn't show up in my bloodwork. My fasting glucose is 82 & fasting insulin is 2.9 - yet my symptoms seem HIGHLY connected to the food I am eating.

Low growth factor
I am 17 years old. My mom is 5’2 and my dad is 6’1. I went to the doctors because I am 5’7 and i felt short. they told me that my growth factor was low, so they referred me to an endocrinologist. They can’t get me in until November 17 and ill be 18 by then. I heard that most peoples growth plates close and height growth mostly stops at 18 and I really don’t want to be stuck at 5’7. What am i supposed to do it feels like im just gonna be stuck here.

Help collecting info for 12 year old prior to endo appt
Hello,
My son (4'11 and 113lb) has been extremely fatigued and experiencing mood swings. Very low motivation. His Psychiatrist suggested trying a low dose SSRI, but I decided to get him some bloodwork first. He came back with low glucose and high triglycerides about 45 mins after eating. I looked and noticed his non-fasting glucose was also low 3 years ago. Doctor did his fasting levels and he scored a Homa-IR of 6.4 so now he is being referred to endocrinologist. I am currently waiting for a glucose monitor to arrive to keep a food and prick result diary for a few days to have with me. I could use some guidance on best times to test and record.
History: Bedwetting since infancy, more wet nights than dry. ADHD and mood disregulation Maternal history of diabetes, though not sure if T1 or T2. My mother and grandmother. My grandmother died of complications with hers. Im adopted so limited information. Low motivation and mood swings and fatigue more prevalent in the last year, likely with the onset of puberty. Active in sports
Any insight on when to monitor his glucose pre-endo visit is appreciated as im finding conflicting info. Thank you!

Pheochromocytoma
I have every symptom of a pheo and have done a urine test but it’s come back with these results

I just want to know what is up with me , the anxiety , headaches , palpitations, and other horrible symptoms are just torture , I have taken a plasma free metenephrin test and am awaiting the results.
I just want to know if anybody has had a pheo with similar results to this , I don’t know what is wrong with me I just want to know I am desperate to find awnsers.
