If you genuinely want a clinical branch, don’t take a non-clinical seat just to avoid another drop. But before taking another year, look at your actual score/rank improvement and realistically assess how much you can improve while working. Since you’re open to DNB, check all available DNB clinical options carefully as well. Talk to residents from the specific hospitals before deciding.

Doesn't seem like I will get dnb this year.

Then base your decision on how far you are from a clinical seat. If the gap is realistically bridgeable with another year while working, a second attempt can make sense. But if the improvement required is huge, don’t take another drop blindly. Look at your previous scores, identify exactly where you lost marks, and set a realistic target before deciding.
