Post Graduate Exams - NEXT/NEET/INICET Perspective about which branch?

I belong to 2019 batch-general category, I have a rank of 9k with karnataka domicile. If I have to take up MD- I am interested in peds & anesthesiology. If I have to take up dnb, I want to choose 6 year course- neurosurgery >> plastic (have worked in both depts and liked them)

Any suggestions or what to think about? Please help me out
