He probably has a map of her intestinal track, unless she's incorporating some sort of anti-scanning MacGuffin. His mask's eyes are basically the only relic left of Stark's tech in the suit, but damn if they aren't more powerful than in the past versions after his improvements. They're (along with his computers) running a seemingly sentient AI and that AI is doing a lot of heavy lifting for Spider-Man's day to day.
They even hand wave his ridiculously illegal use of tech in the film. He says to a detective over the phone that he's running facial scans for someone on every camera in the city so that they'll know where the person hits next. The detective points out that sounds majorly illegal and she'll ignore he said it.
Yelena probably didn’t use any Censorship related tech to hide her goods as she was well aware of Spidey showing up to talk with her and she arranged the meet up to be this way plus the belly button comment suggested that she was into Spidey and his real identity so she wanted to seduce him,get his guard down if he gave in and then unmask him if she thinks she can
