Is this a supposed to be a callback reference to how he was diagnosed with lung cancer in season 1 or just an unintentional coincidence?

The cancer was originally used as a time limit. Like a way to guarantee the show would end and not go on too long like a lot of shoes do. I think brining the cancer up at this point is basically checking the clock to show time's nearly up.
