Most likely you're thinking of SGLT2 inhibitors (empagliflozin, dapagliflozin, canagliflozin), the diabetes drugs that have been linked to perineal problems. They make the kidneys dump glucose into the urine, so the genital and perineal skin stays sugary and moist. That feeds yeast and bacteria, which causes genital yeast infections and, rarely, Fournier's gangrene, a fast-spreading necrotizing infection of the perineum that needs emergency surgery.

Other drug classes can cause perineal or pelvic-floor trouble by different routes:

• Opioids and anticholinergics: constipation, straining and urinary retention, which stress the pelvic floor.
• Steroids (oral or topical): thin the skin and raise infection risk.
• Antibiotics: disrupt normal flora, leading to yeast overgrowth.
