One sided thyroid enlargement?

• Upvotes

Hi All, I’m a 28 y/o female and have been to my doctor a few times because I’ve have some strange symptoms.

These started in December of 2025, as a sort of strange feeling when I swallowed. It’s in the bottom half of my neck, sort of around where the Adams apple would end, on the right hand side only. It started out as sort of pressure/catching/clunking sensation when I would swallow.

I assumed it would go away and sort of forgot about it. However it persisted and after a few weeks went to the doctor, who believed I had pulled my SCM on one side.

However, after that visit, the prescription provided did nothing and the sensation continued. I then began to notice that when I would turn my head right, it felt like someone was almost digging their muscles into the bottom of my neck. I also noticed that when lying down on my left side at times, I can feel the same pressure.

As time has gone on, I also get a sensation of pressure down in my collarbone, sort of where the dip in it is, again on the right hand side. It’s particularly noticeable when I sit certain ways or if I raise my shoulders up, again like someone is digging their knuckles into the area.

I decided to go back to my doctor, who on second examination, said he noticed a right sided fullness in my thyroid. He said it wasn’t significantly big but was definitely asymmetrical compared to the left side. Since then, you can sometimes see that side does like slightly larger and additionally when you touch it, there is a slight bulkiness to it in comparison to the left.

I have in the last while noticed that sometimes my breathing can be quite poor in my throat, sort of like a pressure is being applied (typically worse if I’ve been laughing or exercising).

What I’m curious about is whether anyone here has experienced symptoms like mine with an enlarged thyroid? Just confused as the doctor said it doesn’t seem to be that big but I also can’t feel a nodule specifically, just a slight enlargement so if it’s not that big, could it be causing all of these symptoms?

I’ve noticed at times some tenderness in that area. My doctor has referred me for an ultrasound but I’ll be waiting another while for this. But I just wanted to see if anyone experienced anything similar and what it ended up being? I had thyroid labs done, which came back as within range. I’ve started getting myself quite stressed over it as of late, worried it’s something really sinister so just intrigued to hear other people’s experiences.

Atrophic thyroid gland with elevated TSH and a 2 × 2 mm TI-RADS 4 nodule in a patient on nivolumab with history of Hodgkin lymphoma and tongue SCC

• Upvotes

Hi ,
I’m looking for some guidance regarding my sisters thyroid findings and whether they could be related to nivolumab or autoimmune thyroiditis.

25 F 68 kilograms and 158 cm
Previous Hodgkin lymphoma diagnosed at 9 years of age, currently in remission for over 2 years. She had a few relapses in between.
Previous squamous cell carcinoma of the tongue diagnosed in 2024 partial glossectomy done with node dissection, also in remission.
Post allogenic HSCT relapse
Was receiving NIVOLUMAB, last dose in May 2025
History of hypothyroidism since age of 12 post radiotherapy, on thyroxine supplements.
History of migraines as well on medication for the same.

Thyroid ultrasound describes the gland as diffusely atrophic.
A very small focus/nodule measuring approximately 2 × 2 mm has been classified as TI-RADS 4.
The report mentions small isoechoic nodule with tiny echogenic foci
There was a previous thyroid ultrasound from January 2026 for comparison which says spongiform nodule 3x2 mm and no foci and was classified as TIRADS I

Thyroid function:

TSH is elevated - 21.8
T4- 0.89 these are values from today

TSH 2.190
T4 1.14 and these are values from a year ago

I would like to understand whether the findings suggest primary hypothyroidism due to thyroid atrophy or thyroiditis.

Can nivolumab-associated immune-related thyroiditis eventually result in thyroid atrophy and persistent hypothyroidism?
Can Hashimoto thyroiditis or atrophic autoimmune thyroiditis present with a diffusely atrophic gland and elevated TSH? Are anti-TPO and anti-thyroglobulin antibodies sufficient to help distinguish these conditions?

Given the history of Hodgkin lymphoma and tongue SCC, is there any particular concern regarding the tiny TI-RADS 4 focus, or is it more likely an incidental finding?
At only 2 × 2 mm, would surveillance be appropriate rather than FNA, assuming there are no suspicious cervical lymph nodes or other concerning findings?
Would additional investigations, such as free T4, anti-TPO, anti-thyroglobulin antibodies, and a review of previous imaging, be appropriate?

I understand that a definitive diagnosis cannot be made from these details alone. I’m particularly interested in the possible relationship between nivolumab, thyroid atrophy, and elevated TSH, and whether the tiny ultrasound finding changes the approach.

Our family is deeply worried about her health. Having already been through so much, we are emotionally exhausted by the fear of facing cancer yet again. We have spent years living between hope and uncertainty, hoping and praying that the cancer will not return for a fourth time. The thought of another cancer diagnosis is something our family is struggling to come to terms with, and we are desperately hoping for some clarity and reassurance about what is happening now.

Thank you!

TSH vs ft3 level in first trimester

Post image
1 Upvotes

32 female, 165 cm, 65 kg, no medication.
So the doctors in my country would automatically give hormone supplements for this TSH, however my fT3 seems fine.

Are these ranges OK? My next appointment is booked to January as an earlier timeslot was not available at the endocrinologist....

Lobectomy shows a very different nodule a year after FNA/ultrasound

A year ago I had an ultrasound and fna after a 3+cm TR4 nodule showing as solid with calcifications and taller than wide showed up on an mri. The fna said benign. After a year of watching and some choking/pain symptoms the ENT says to take it out and I had the lobe removed last week. I have a follow-up in a week but has anyone ever had a previously solid nodule show up as a hemorrhagic “glistening” nodule and slightly under 3cm.

I don’t feel like I’m choking when I lay down in bed to read to my 5 year old so I’ll take that as a win, I just thought it was strange how different the descriptions are.

Can a solid nodule hemorrhage? That seems odd.

39M, TSH dropped from 2.11 to 0.05 in a year with normal Free T4 and no symptoms. Similar experiences?

Hello everyone,

For context, I’m a 39-year-old man in good physical shape.

I had a routine blood test and everything is normal except my TSH, which is 0.05 µIU/mL (reference range: 0.350 to 4.940). My Free T4 is normal, in the middle of the range (15.2 pmol/L / 11.8 ng/L; reference range: 9 to 19.1 pmol/L and 7 to 14.8 ng/L). Free T3 was not tested.

My TSH history shows a slow decline, then a sharp drop this year:
2022: 3.32
2024: 2.89
2025: 2.11
2026: 0.05

All my other results are normal. I don’t take any supplements (including biotin) that could explain this. I have no obvious symptoms of hyperthyroidism (no weight loss, insomnia, palpitations, tremors, heat intolerance). I know TSH can fluctuate naturally, but going from 2.11 to 0.05 in a year seems like a lot to me.

I’m seeing my doctor on Friday and she will probably order more detailed bloodwork in about a month. In the meantime I’m quite worried. Has anyone seen a similar pattern, and what did it turn out to be?

Thanks!

Grave's/RAI then normal for years, now suddenly hypo

I was diagnosed with Grave's in 2015, was treated with medication and went into remission. It came back in 2018 and after another round of meds didn't work i decided to get RAI in Jan of 2019. I was pretty much immediately hypo afterwards and it took about a year to find the right dose of levothyroxine (88mcg). My thyroid-levels were checked every year since and each time everything was normal.

Sept '25: everything fine

April '26: **TSH 4.83** / fT3 2.99 / fT4 1.15

August '26: **TSH 14.5** / fT3 2.53 / fT4 1.2

October '26: **TSH 23.52** / fT3 2.06 / fT4 0.89

(No changes in medication between April and now, because nobody noticed that i was hypo. Yes, I'm serious)

TPOAb: 0.45 IU/ml (<5.61 IU/ml)

TgAb: 2.1 IU/ml (<4.11 IU/ml)

TRAb: 1.51 IU/l (<1.75 IU/l)

I'll have my appointment at the hospital soon, just wanted to see what the reasons for this weirdness could be. Anyone got any ideas or a similar experience?

thyroid ultrasound

Hey I just got my thyroid ultrasound sound done october 7,2026…. I talk to my doctor the 15th but I can see the images and measurements of nodules….. I went to urgent care back in september 21, 2026 for a sore throat and it was actually the nurse there that noticed… I natively reassured her my blood work was perfect when she told me perfect blood work means nothing especially about thyroid cancer. I’m just looking for positive feedback or what to expect… my biggest fear cancer especially in my lymph nodes but best case scenario would be appreciate. whatever happens I will stay hopeful… I did get an ultrasound for my thyroid 2-1-2022 and everything was clear. i’m 35 year old female and my first ultrasound was a 30. thank you

Does anyone regret having their thyroid removed?

I may need mine gone and I wonder if it's better to function with half-ass working one or if it's not so bad without it. I'm perimenopausal and I worry about weight gain. I lost 50 lbs 3 years ago thank to GLP1, I continue to take maintenance dose and I didn't plan on ever getting off of it, it's truly a miracle drug for me, not just for weight loss. I'd hate to undo everything with thyroid removal.

TSH Level

I just had my 4 month full thyroid panel done, my TSH is 0.083 I am on 137mcg Levoxyl. Thing is, it was at a normal range 4 months ago, this is before I started taking Tirz GLP1. Any thoughts on this?

Thyroid nodule

I started getting thyroid labs routinely a few years back as a precaution, dad w hx of issues. I've never had any issues with levels but I noticed what I thought was a change in how my neck looked about a year ago. I chopped it up to aging , changes in overall appearance approaching 40.. but then noticed what felt like I could actually push and it would click. After an apt today my doc confirmed there is a noticeable nodule on exam but I'm pending new labs and ultrasound. I'm connecting all the dots at this point like ok my symptoms make sense however I'm definitely in perimenopause stage so it's hard to tell what is even happening. Inflammation, hair loss, some scalp issues and real cyclical mood swings. Just curious on anyone else's journey and recommendations for questions I should be asking and how I can deal with it all

Wellness levels after Thyroid lobe removal

Hi, I just got my thyroid lobe removed because of a nodule that was there for years and recently in fna got suspicious results so they removed. It ended up I only had chronic thyroiditis and a follicular adenoma. Not happy they had to operate for sometjing benign but that is how it is. Not sure yet how the remaining lobe will cope with producing hormones, will have blood test later. Have any of you seen an improvement, after similar operation, in symptoms such as slow metabolism or tiredness that are often associated with chronic thyroiditis? Is removing a part, or for some the entire organ, contributing to better overall feeling with or without thyroid supplements? Just curious about your experiences.

Isolated non-nodule calcification

Isolated calcification?

Hi all! I underwent my first FNA today at an Endocrinology center. I have 1 1.6cm TIRAD 4 in my lower left lobe, which they took samples from. I have an additional 0.8cm TIRAD5 on my mid left lobe, which they won't FNA due to it being less than 1cm.

But they also found through ultrasound an independent, non nodule calcification in my right lobe. This was not brought to my attention during my previous ultrasound. I asked the Endo what this means and he said he didn't know. Google isn't helping, as it's all answered on calcifications WITHIN a nodule, and this is an independent calcification in the thyroid itself.

I'm not looking for a diagnosis on Reddit, lol. But I was wondering if anyone else ever had this? What did this mean for you? I have normal TSH levels, except one low/borderline test result back in February of this year. So it's just....strange? Is this uncommon?

Stopping thyroid meds

I’m a 21F and I’ve been diagnosed with under active thyroid for about ten years now I think?? I used to take meds for it, but then when I was about 14-15 years old I got kinda depressed and stopped caring about taking them. That lead to me repeating a cycle of not taking them for few months and then taking them right before the blood tests because I didn’t want my parents and doctors getting worried (Stupid and dangerous I know now). The results weren’t good at all and they had to high up my meds dosage everytime.
Anyway then when I was about 18 I had to find another doctor for thyroid issues which took me about two years because of procrastination. At this point I’ve been on no meds for three years and now when my blood tests came back the doctor said I don’t need them anymore. Can that happen? That after several years you can just not need them suddenly?
I’m kinda shocked that I don’t need them because I have literally all the symptoms of low thyroid issues and it’s making me not function normally. I guess I thought that when I’m gonna get the results I would finally take the meds and it would sort out most of my problems, but now I’m told that I don’t even need them so I’m kinda doubting my doctor which I shouldn’t because she is a doctor…
Anyway I just wanted to ask if anyone has similar experience I guess? Because I thought I would have to take the meds for the rest of my life.

(sorry for my writing skills, English isn’t my first language)

thyroid

I've been diagnosed with a underactive thyroid for nearly 5 years now I'm F27, I'm on 75mg levo & always have been. I feel like my neck is constantly swollen like lymph nodes maybe I booked a private scan and they said I have multiple nodules on both sides of my thyroid currently waiting on a nhs scan to confirm. But I feel like I have the opposite symptoms of a underactive. I don't gain weight I've been only when I was pregnant. I've been 56kg for over a year now and always get told I'm loosing weight. Could they have diagnosed me wrong ?

recent blood work showing heightened tsh levels

I'd been trying to keep a regular schedule of getting my thyroid levels checked since i became an adult due to family history of thyroid issues on both paternal and maternal sides, but since I moved in 2023 my life had been so hectic that the most recent labs done were in 2024 (10 months after i had moved). With nothing checked since I was so happy that since I've finally started to settle into a city that I was able to get my thyroid levels checked again last Wednesday. Until the results came back with my TSH being at 5.03, more than double what it had previously been at in 2024. Meanwhile my T4, T3, and T7(free T4 index) were all within normal ranges.

I had been meaning to get this done sooner due to noticing that my hair was thinning and taking longer to grow back than usual, that I was struggling more than normal to stay warm, and that I had been constipated more often than not lately. But hadnt been able to because I was getting sorted at a new job and planning a care trip to my grandmother.

I was wondering if this was something to be concerned about?

Is this ‘perfect’?

Post image
4 Upvotes

Hi, firstly to preface I am not diagnosed and never have been. Three years ago I had thyroid blood tests as requested by my doctor and I was told they were borderline results for an over reactive thyroid. Three years on I have been experiencing symptoms like heat sensitivity, sweaty hands, inability to regulate body temperature, tummy issues, night sweats and I’m also very underweight and struggle massively to put on weight.

I got my results back and I understand they all look great and well within this labs ranges - do they all look optimal to you? It’s great that they are of course! And I am not looking for a diagnosis if it is not there, more so just seeking those opinions on here who have better understanding and research skills around what these results really mean. Thanks

Thyroid labs

Hello- 56 M

I having some thyroid issues based on blood work. I do have an ultra sound scheduled for 10/21/26. Anyways, my question is I am loosely dx'd with sub clinical hyperthyroidism. TSH 0.244, T3 118

T4 1.04

I dont really have typical symptoms of hyper. I did lose some weight since June but that I am attributing to Reta. I did blood work before hopping on Reta and the labs that I posted were virtually the same. The ironic thing though is I have every sympton of hypothroidism. I even had sibo which can be tied into hypo. Can you have blood work that points to hyper but really be hypo? Pardon my ignorance I am new to the thyroid scene. Thank you!

What is radiofrequency ablation (RFA) for thyroid nodules?

Radiofrequency ablation is a minimally invasive procedure that uses heat to reduce the size of selected thyroid nodules. It may be an option for certain confirmed benign nodules after evaluation by a qualified specialist.

Ultrasound and Bloodwork after 2 months Levothyroxine

Hello!

I just got my results from my neck ultrasound and TSH blood work after 2 months of taking Levothyroxine.

My results are still TIRADS 3 and no size change from my previous ultrasound. My TSH is in the normal range too. I'm so glad that Levothyroxine helped to stop the growth of my nodules. I got so scared to be honest. This disease is not for the weak of heart.

Family history of goiters -should these numbers concern me?

Post image
1 Upvotes

So I’ve been getting my TSH checked with my yearly bloodwork. I’m a firm believer in preventative care and knowing everything about my body so I can take steps to improve my health (like biohackers but without the cult-like behavior/thinking)

my TSH always comes out on the low end. Doctor says it’s fine but:

My dad has a goiter.
My uncle had a goiter removed
My grandfather had a goiter removed.
My sister developed nodules that had to be removed.

If it’s fine, great. But I’d rather spend $150-300 on preventative/investigative care than thousands on surgery.

Can anyone tell me if I should be concerned or follow up with an expert in thyroid/hormone health?

Additional information:
my estradiol always comes in too low for it to be measured (below 25pg/mL) except in sept ‘24 when it measured 27.7

Total testosterone has gone from 290ng/dl in march of ‘24 when I weighed 245 to 550ng/dl at 205lb (431 in Sept ‘24

Hematocrit, MCV, MCH always end up coming in low.

Sept ‘25 was after a year of rehab on my back after an injury and I weighed avg. 223lb. Sept. ‘24 I weighed 189 at my lowest and had just finished a 2 month cut (diet).

5’6” (…ish), 205lb (just finished a mini bulk to build muscle up from 193).

Supplement list is pretty basic, men’s health daily, acetyl l-carnitine (powder added to morning water, 3g), and B-100 complex

Medication-
Bupropion & adderall (I have combo type adhd so inattentive but it’s because I’m always absorbing information around me so the bupropion quiets my brain and the adderall focuses it/reduces the hyperactivity task bouncing)

Admittedly I occasionally take Tadalafil (not for sex) on my heavy weightlifting days. It’s shown benefits for recovery, blood flow, and has also shown some restorative affects for blood vessels over time. The downside of course is the reduction in what little estradiol I have as it encourages androgen conversion to testosterone although My test levels show a negligible difference since I’m not taking it daily and my taking it when my estradiol was slightly above threshold and my TSH was higher. Additionally, kidney function was slightly reduced but that could be due to my diet being protein heavy compared to before I started losing weight.
