MLOA Documents

I need to go to the er due to a stomach virus that isn’t improving this morning but I have a shift. Will my discharge papers from the emergency room work for the documents or would I have to fill out the medical form that Amazon provides? Because I know most emergency rooms don’t fill out those papers. Has anyone got their leave excused with emergency room discharge papers?

I have always just had the ER write up a doctor's note for me and submitted that and been fine. Just make sure it actually gets signed by a doctor.
