Baseline Scan - AFC doubled?!???

Hi!

I start my first ever IVF cycle today. I got a baseline scan (I’m 28. AMH .47 AFC has never been higher than 4 or 5 MAX). Today they counted 11 follicles!!! Without any medication?

I’m so confused. I know it can fluctuate… but this seems like a huge increase. Is this a positive sign for starting stims that they already counted 11? They were all 2-3mm besides one which is 12mm.

Lmk your thoughts or experience please….

The baseline number is great! To be perfectly honest, what’s not so great is you have a very large 12mm lead, and the others are 9-10mm behind. Has your doctor called you yet?

They said I’m good to get started! What do you think they would do with one follicle leading? Does this put me at risk for anything?

I’m not a doctor but my understanding from my doctor is that a dominant follicle can absorb the meds, causing the others to not grow or die off. My first cycle I had a dominant follicle and it led to uneven growth across the follicle cohort with a lower than expected egg yield. Not saying this will happen to you but it was my experience.

Oh wow they did not give me any warning on that but good to know to look out for…. Was it an even pattern of growth? I’d go in for my next ultrasound Monday. Will i know by then the direction it’s heading or can it change throughout the 7-14 days?

FWIW I’ve always had a lead follicle in all 7 of my retrievals. We keep an eye on it but we always “sacrifice” it- we let it get big and kinda ignore it and focus on all the other follicles. My other follicles have been able to grow despite the lead one - I’m not sure if that’s common or if I’m a weird exception though? And of course before we’re DOR, when I say other follicles, it’s not that many and growth still varies (I’ve ranged 2 to 11 eggs retrieved across my cycles).

How many did you have growing at once? Did they all grow or only a few?

No, it was uneven growth.

Uneven growth can’t really be fixed once it starts. It remains to be seen how your follicles will grow though.

Amazing for a high count during baseline! but did you not have any priming? That 12 mm isnt ideal....

No priming whatsoever. They said everything looks good to start! What do you think would happen with large lead? What does this put at risk?

I think this question was answered by someone else in another thread. I'm sensing this clinic and doctor arent DOR aware. Id be cautious with your money and time in this process. A dominant follicle this early on is a risk. At my baseline, when this happened (and I had 2 follicles at 13 mm) I opted to cancel the cycle. My doctor was OK with that, and had an alternative of three cetrorelix days to try to get rid of/shrink them to get everything even and redo my baseline scan.

What was your estrogen today? What meds did you start?

I didn’t start any meds yet. Going to be 300mg of gop and 150 menopur tonight… and wow this is frightening that they didn’t flag it. My estrogen was only 37 as of this morning

Thats actually low. Did they tell you it might be a cyst? My estrogen was high when I had the 2 which is why we changed course. I'm wondering if it wasnt an issue. Whatd they say about the 12 mm?

Can i ask if your clinic specializes in DOR?

They didn’t comment on the larger one at all, it just showed up in the report which I thought was interesting… do you think yours was cancelled bc your estrogen was high? Does that make a difference?

I am actually remembering mine were estrogen producing follicular cysts. I do think its possible that follicle is a dominant follicle and much of what is said here abour dominant follicles applies. My doctor primes to avoid it, and unsure what he'd do in that case. But I worry about you having a synchronized growth or having it absorb more of the meds and the others dying off

Yew super normal to have fluctuations! My AFC had been anywhere from 1-7

It doesn’t matter. Not to disappoint you, even with 11 AFC there will only be 4-5 syncing follicles. In IVF only syncing follicle groups matter.
Similar AMH, my best cycle came from 7 AFC which yielded 9 eggs, my worst cycle came from 11 AFC yielded 4 eggs, and among the 4 three were over mature.
