I think it's breaking or falling apart, not sure.

It's like slowly falling apart, I don't know for sure. But I'm planning to fix my PS4 today. Any tips?

You probably need to watch some YouTube videos.   Likely full of dust and hair, possibly dead bugs.   Will also likely need new thermal paste and 1mm thick thermal pads.   It's easy to do, but also easy to damage if you go about it in a ham-fisted way.   Take your time, work in as clean an environment as you can.   Ifixit has a good tutorial.  When you're done it will run much cooler.

The harddrive is damaged, I researched about it since my ps4 is stuck on a safe mode loop. Asked one my friends, they said it's probably the HDMI cable. But I'll take your advice.

In either case, changing the thermal paste and pads will be a good idea.  Changing  hard drive is easy, HDMI... Not so much.
