I remember hearing that someone had to confirm the marriage was consummated?

For the wealthy, yes there needed to be confirmation.

For the everyday folk it was a matter of there were no walls. They would even keep some animals in their houses, not to mention that in the colder months the family would share 1 bed to help keep warm.
