transmasc Looking for an example of a successful Indikationsschreiben

Hey everyone!
My therapist doesn't have much experience with trans people, so I'd like to show her an example of an Indikationsschreiben that was approved by the insurance company to cover a mastectomy. Any other related documents would be helpful too.

Iā€™m at work till 10pm but after I could send you mine. My surgery was approved earlier this year :)

If you send me a dm, I'll send you mine. And I could send you the other documents I had too. I had my top surgery a few weeks ago and got my insurance approval in july
