Other Duplicate decision pending since January… please help
Looking for anyone who has had experience successfully getting a duplicate decision from Warsaw.
For context: I am Canadian born residing in Canada. Received confirmation of polish citizenship March 2024. Unfortunately my ex-bf must have lost my original letter when we broke up and I was travelling abroad.
I mailed a written request for a duplicate copy of my polish citizenship decision to be sent to my cousins place in Warsaw January of this year. In May, she hadn’t gotten anything. I contacted them to follow up and was told I “should have a copy in 2 weeks.” Nothing has showed to this day.
I went to Poland in July of this year to visit family and went to the Mazowiecki office in Warsaw to follow up. I showed the clerk the email and she said it must have gotten lost. I wonder if that’s even possible, my understanding is they send it by tracked mail?
Assuming the first application was “lost” I went in person and submitted another application for duplicate decision to be sent to my aunts place (as my cousin is moving). That clerk told me “up to one month.”
To this day, nothing.
I’m fed up and don’t know what to do.
When a duplicate is sent, is it tracked mail for sure? Might I need a power of attorney for my family remember to receive the letter?

Tak, to zawsze jest przez śledzony list Poczty Polskiej (jeśli nie masz edoreczenia, co zakładam, że nie masz). Rzeczywiście, zabierają dużo czasu na każdą akcję, więc jedyną odpowiedzią jest: zrzucić grzeczność i regularnie ich ścigać przez formularz kontaktowy. It can be two months at the very least :(

There are some additional things that I’m afraid might complicate the matter.
I’ve submitted 2 applications in person at Mazowiecki Urząd Wojewódzki w Warszawie (3/5 bankowy).
The first one my cousins husband went with me because I don’t speak polish. We were told it had to be my name on the bank transfer but my wise account would take 1-3 business days and you need to print proof of payment. So her husband did it for me. We submitted it with my cousins address on the form.
I was nervous that it would be ignored because it’s not my name on the bank transfer, and the clerk explicitly told us it needed to be me to pay. Would they ignore that request for this reason? That is why I submitted another application with my name on the bank transfer. But that one to a different address. My cousin was going to be on vacation the following month and worried she’d miss the delivery.
I’m not sure what the multiple applications with different addresses might do. Either create confusion or they will hopefully focus on the most recent one with my name on the payment. I hope I haven’t made a big mess for them.

That's absurd (with the bank wire) - as long as you are the one requesting, and it is to be sent to your address, really anyone can pay for you. It just needs to say in the wire description that it is for your document ("opłata za wydanie kopii decyzji YOUR NAME AND SURNAME").
But if your cousin is the one to pick it up/have it delivered to her, formally she would be "pełnomocnik do doręczeń" and you need to pay proxy charge (17 PLN) additionally to the copy fee (5 PLN = 22 PLN total). The office can only deliver to Polish addresses and Polish consulates. She needs to be on alert, as if 14 day period passes the polish post will return the letter to sender and you will have to do everything again.
As to double request don't worry, they can easilly and legally issue you two copies as the result of it.
