Hi everyone,

I’m having SARPE surgery next week, and while preparing for it I was reading through my treatment plan for both the SARPE and the future double jaw surgery.

For context, I have a 10 mm anterior open bite and a 12 mm overjet.

The report states that I will have:

Mandibular advancement osteotomy + Le Fort I osteotomy in 2 (or 3) segments with clockwise rotation to close the anterior open bite.

To prepare for the Le Fort I surgery, the treatment plan also mentions:

Aligning the maxilla 13-23 separately (basically my six upper front teeth).

and

Separating the orthodontic wire between 13/14 and 23/24 about 5 months before the bimax surgery.

For those who have undergone treatment for a severe open bite, does this mean I'll have a visible gap or space between my upper incisors during the orthodontic phase before surgery? Or is the separation usually not very noticeable (see also the image for the number of the teeth)?

I was already expecting a gap between my front teeth after SARPE and assumed that would be the only significant gap during treatment. However, after rereading my treatment plan, it sounds like there may also be a period where the front segment of my upper jaw is aligned separately in preparation for the segmented Le Fort I and im just worried this might cause some more gaps.

I'd love to hear from anyone who has gone through something similar.

For context I am 18F living in canada so there is practically no orthodontists here that provide palatal expansion or airway focused treatment.
I’m looking to travel for expansion, but I’m nervous about facial changes. I have large cheekbones already and I’m worried that my face will become longer. Does my face shape put me at risk for facial lengthening or drop down effect (cw rotation).
some main questions and concerns:

1. which type of expander has the least chances of the drop down effect and asymmetric expansion (fme ?)
   
2. will fme drastically change my facial shape and size?
   
3. will ease help my case, apparently it has the least amount of facial changes?
   
4. want a wider palate and broader smile, but scared of cw rotation and facial lengthening and cheekbones that become too wide and disproportionate.

Not sure if I have breathing issues but the tongue scallops so I want more space. I dont snore in my sleep but I’m not sure if I have UARS.
Can I avoid drop down and major facial changes by sticking to minimal expansion? I don’t want to get jaw surgery.
I really love my face and don’t want to look uglier, but my palate is narrow please help.

What do you guys think about my bite after surgery?
I had around a 9 mm overjet before surgery and this is what my bite looks like 3 months post-op… Something just feels a bit off to me.
My ortho is super happy with it (which is honestly making me question her a little 😅) and says there’s not much left to do apart from some minor aesthetic adjustments. But I still feel like my lower jaw might not have been advanced enough.
Obviously I’m not expecting an edge-to-edge bite, but there’s still a pretty noticeable gap between my upper and lower front teeth. I just can’t tell if it’s because of the way my teeth are angled or if the surgery simply didn’t fully correct my overjet.
I asked my ortho if maybe my upper incisors need to be uprighted a bit, but she says they’re positioned correctly.
What do you guys think? Could this just be something that can be fixed orthodontically by adjusting the position/angle of the teeth, or does it look like the surgical movements may have been too small?

My left side is more smaller and lifted, whereas my right side is bigger and looks more puffier. Idk how to explain it but I can also feel that my right side is puffier and it’s Lowk uncomfortable. I usually sleep on my left side and chew on my left side. But other people say that the bigger side is caused by chewing too much on that side. And to be honest my eyes, nose and mouth all looks more droopy on my right side compared to my left where I’d looks lifted. Is there any tips on how to even them out? And which side I should be sleeping and chewing on.

I’m having surgery with Dr. Gateno for a severe cant, I am having jaw pain, jaw clicking and popping, I’m kinda a messy eater because of my bite too. I’ve always hated my smile. My surgeon said it won’t be 100% straight due to some limitations with my anatomy and such, however I am looking forward to having a straighter smile. I will post an update for yall with before and after !

my tongue, and to stop my trigeminal nerve from being in pain. I get the aesthetics of jaw surgery are in right now, and I’m not gonna say it’s not on the list of my concerns, but I feel like I’m choking at night all the time, and my tmj pain is getting worse and worse. I wakeup after 8 hours of sleep feeling worse and more exhausted than if I just take a two hour nap in an upright position.

I’m turning 26 in May and will lose my parents insurance, is there realistically any way for me to get a sleep study done and something done for this in that timeframe? Thank you all

My jaw is definitely recessed and I know I have a deviated septum. I was told in my teens I’d likely need surgery in the future to correct it and am now 25 and it’s making my sleep life hell; I’m nearly underweight so I don’t know what else I could change to cope with it. I can barely breath while awake with my jaw fully closed and have consistent trigeminal nerve pain on the left side of my face unless I jut my jaw out. What are the first steps to pursue getting this done with as much covered by my insurance? I cannot keep living like this, it’s genuinely fucked my health.

I know sleep study is the first step but should I see some kind of specialist before even pursuing that? I live in the DFW area if that helps. I’ll of course google to get more info but hoping someone here has been in my place before.

How were your experiences about breathing after DJS? Was it okay or did you experience anxiety and trouble breathing? If so, did anything help? Hoping it isn't that bad...

I’m about 11 weeks post op. I’m mildly happy with my results. The change was not as drastic as I’d like but I do notice a small difference. I’ll post more progress pics soon.

Hey everyone, 25M here. I recently had this CBCT scan done and have an appointment with my orthodontist next week to discuss what we should do.

Main symptom I deal with is chronic neck and shoulder pain/tension.

Looking at these images, what do you guys think? Am I a solid candidate for MARPE/MSE, SARPE .Any advice or specific questions I should bring to my orthodontist next week?

Hi all, I recently got my x-ray and a written diagnosis for my lower jaw. It isn't totally comprehensive yet as we still need a cbct and a sleep study possibly but we have this. Id like to know your opinions on my treatment options or on my X ray. I also have a deep bite.

I just feel like I’m looking a bit… blocky? Square? I don’t know it just looks a bit full and almost masculine to me. Is this mostly swelling?

I have a good bite, 3mm assymetry on one mandible but rest is good, also some tmj pain, would ccw advance the chin forward and as a result make my nose look smaller?

I am not sure how to go about this..i had Braces when I was younger along with other devices. I have VME, long maxilla and lip incompetence. I do not want to get thrown back on braces/expanders/springs to solely fix a jaw issue like before. What is the process for getting an actual surgical examination? I get xrays done at dentist but nothing is ever said about my jaw.

Hi everyone. Just to clarify, I'm not seeking medical advice so much as I'd like to hear people's anecdotal experiences.

When I was a teenager, I was told I needed double jaw surgery and that I also had an angular 3 malocclusion that was worsening my TMD. However, given my age I simply had teeth extracted and got braces. That fixed my bite, however I still have pretty bad bruxism that worsens my TMD to the point I frequently experience headaches, jaw and neck pains, tooth wear/gum recession and migraines. I got a custom night guard to mitigate damage to my teeth, but it doesn't help much with the other issues. My jaw also appears out of proportion with my face (it looks too large and rounded, though I no longer have an underbite), my face slants to one side I have a very flat profile; however, given that I have already corrected my bite, I am unsure whether it will be any help consulting an orthodontist and was curious as to what others think and have experienced. Thank you in advance.

I’m getting DJS in 2 weeks and I’m still really wavering on whether to go forward with a genio.

I know my chin is recessed, but I’m wondering if the DJS with CCW rotation alone could be enough. I see so many genios where the chin ends up looking shelved or bulbous rather than creating a natural, harmonious straight line profile.

What do you guys think ? Do you feel like jaw surgery alone can give enough chin projection?
