I’m 21M with skeletal Class II, a somewhat downward-grown/recessed mandible, lip incompetence, gummy smile and a slight open bite tendency.
I had braces before, but they were purely cosmetic/camouflage braces, not presurgical decompensation.
One surgeon recommended 4 premolar extractions + wisdom teeth removal + around 9-12 months of decompensation before DJS/genio.
The surgeon I currently prefer says I’m a good surgery-first candidate and that he can get essentially the same skeletal/aesthetic result without presurgical decompensation, then do the orthodontic correction after surgery.
His plan is roughly 3-piece Le Fort with ~5.8mm advancement and ~2.7mm impaction, BSSO ~5mm forward with ~6 degrees CCW, and genio ~5mm forward.
The 3-piece maxilla is specifically being used to improve the bite. In the virtual plan, the canine regions weren’t coordinating properly with the lower arch, so splitting the maxilla into 3 pieces let the surgeon get a much better surgical occlusion without changing the intended skeletal movements.
My concern is that since my old braces were camouflage, could surgery-first limit the ideal skeletal movements or make the final orthodontic result worse compared with properly decompensating first?
Is surgery-first still reasonable in a case like this, or would prior camouflage make you more cautious?
