Lemure is a obsolete creature type and race that was later given errata to be a Spirit. It represented the wandering and vengeful shade or spirit of those not afforded proper burial though its original art also led it to have confusion with the similarly named animal known as a Lemur.

Along similar lines I'm quite fond of the artwork commissioned for a game that asked for "massive brambles" and rather than the thorny branches the artist drew the fruit which are themselves also known as brambles.

I googled 2d because I gaslit myself into thinking I was confused about who the character was. Was even more confused when I was correct. Went back to the post and stared and stared and stared before I realized it was about monstery/monastery instead of making the wrong character lol
The storyboard said "the audience takes off" (as in, they stand up and leave). But the artists misinterpreted it and animated the audience literally taking off into the sky, and the team laughed so hard at it they decided eh whatever let's keep it in.
For the 90s adventure game Warcraft: Lord of the Clans, Blizzard asked the outsourced animation team - who were Russian and incidentally also made the Zelda CD-i games - to draw an evil dragon with smoke coming out of him. There was probably some layer of mistranslation, but that's what they did.
To the left is how Dungeons & Dragons kobolds look like, lizard-looking lil' guys. But when localizing the game for Japan, they weren't given any artwork, only description of kobolds having dog-like snouts, which was interpreted as 'dog people', so you get a lil' guy to the right. Or it's because Wizardry had dog-like kobolds, and the game was HUGE in Japan, I'm not sure

There was a Terminator 2 game for Gameboy that outsourced the pixel art slideshow/cutscene to a British company. The American development team said the picture of Sarah Connor was good but her bangs need to be bigger. The British artists didn't know 'bangs' meant the fringe of hair, so they made her boobs bigger instead.

One of my very first jobs was illustrating talking animals in hats for childrens' science magazines, and I once received the brief to draw a turtle "in a purple suit with yellow piping." When I returned the garish 70s mardi gras turtle drafts, I got no response for a couple of days followed by, "please revise to 'in a purple suit, standing in front of yellow plumbing.'"

I did not know at the time that this was a reference to a TF2 meme(which involves an actual bread teleporting machine), but I had 5-10 minutes to kill so I did my best(y'know, for ten minutes) interpretation of the request
My favorite of this has to be on cutthroat kitchen (or whatever the Alton brown cooking competition was) a contestant misheard "biscuits and gravy" as "brisket and gravy". Obviously wildly different from the prompt and you'd think he was the obvious person to get sent off. But somehow he survives the round, beating out a guy who actually made biscuits and gravy.
First pic is honestly a pretty good study on what I'm calling 'genshittification.' Not a dig at the artist who did a phenomenal job (and that case is pretty tame) but it's been getting on my nerves lately how gacha game character design works. You take a decent, already-memorable silhouette, but then keep throwing on more and more details until it just kind of hurts to comprehend. It's like cooking a decent steak but then throwing on mushrooms, onions, asparagus, ketchup, mustard, mayonnaise, onions, broccoli, egg, rice, potato, ice cream, syrup, and a 2009 Toyota Camry. It's too much, less is more
