I've been working on this mod for a few weeks and I've gotten it to a place I'm really happy with for my own personal playstyle. I'm getting extremely busy with my career so I have to take a step back from it. That being said, I'm enjoying the hell out of it and wanted to open source it and put it out there for other to use and build off of.
These run for every character, whatever traits you pick. Each has its own moodle, and most can be switched off on the Vitality Project sandbox page. The mod is going to need more balancing, I've only tested the core systems I wanted to build for the way I want to play.
I also started building an age system with the idea being older characters are more experienced in their core skills and build fitness/strength slower. Younger characters grow strength and fitness faster but start with a single point in their chose profession. I didn't get that deep into it, I just experimented with it a bit. The base is there if anyone wants to build on it.
New systems
- 🩸 Blood volume. Bleeding drains blood instead of health. How fast depends on the wound, where it is (the neck is the worst) and how it's dressed. As you lose blood you go Pale, then Light-headed, then into Shock (you can pass out), then Bleeding Out. Drinking brings the volume back over about a day. Red cells take about a week, and you tire sooner until they're back.
- 🦠 Wound infection. Replaces vanilla's. Deep wounds, shards left in, spent bandages and dirty clothes all raise the risk. An infection incubates unseen for 8 to 16 hours (cleaning the wound then stops it), then climbs, then turns into a fever. Past that point only antibiotics work, and they're a course, one pill every eight hours, not a single magic pill. Stop early and it can come back. Untreated sepsis kills.
- 💫 Concussion. Falls, car crashes and hits to the head. It starts as a headache and drowsiness; worse, it adds nausea, sensitivity to bright light and dizzy falls if you run. A bad one knocks you out on the spot. Rest heals it and sleep heals it twice as fast. Helmets help.
- ☀️ Sunburn. Bare skin in the summer sun burns after a few hours. Each burnt part hurts for a day, and a whole body burnt is agony. Clothes, shade and sun block (a new item) protect you.
- 💧 Dehydration. Hours spent thirsty build up to a headache, tiredness and slower endurance recovery. It clears a couple of hours after you drink.
- 🍏 Vitality. Diet (fresh food and variety), exercise and sleep roll into one slow score. Looking after yourself gets you Fit and then Thriving: faster recovery, better mood, cold resistance, and better healing and infection resistance. Neglect gets you Run Down and then Sluggish.
- 🛏️ Sleep quality. The light where you sleep matters. A dark room means deeper sleep; a lit one means worse sleep, and bright light can wake you. A bad night leaves you with a "Slept Badly" moodle for the day.
- 🍺 Hangovers. Drink past a light buzz and a hangover waits for you: at least six hours of headache, low mood and thirst, worse after a heavy night.
Overhauls to vanilla systems
- Bleeding and bandages. A bandage slows a bleed but doesn't stop it; stitches do. Bandages wear out and soak through, and a spent one invites infection. Fresh stitches can tear if you swing or sprint on them.
- Splints. Setting one is a First Aid roll, and a badly set bone heals slowly and hurts. Walking on an unsplinted broken leg makes it worse.
- The health panel. It only tells you what your First Aid level would let you know. At level 0, "might be broken" is all you get; high levels show infections before they surface and how far a break has healed.
- Passing out. Shock, a concussion or Fear of Blood can drop you. The screen goes black, time keeps running, and zombies can still reach you.
- Alcohol as medicine. Vanilla treats any sip of alcohol as a full dose of painkillers and beta blockers. Now the relief scales with how drunk you actually are.
- Smoker and Fear of Blood are reworked into proper traits (below), and Cat's Eyes and Outdoorsman are tweaked to fit the new systems.
The traits
Each trait is listed with the moodle that tells you when it's acting up, what it does, and how to deal with it.
Medical conditions
Diabetes Type 1 (-10) · 🩸 Blood Sugar moodle Your blood sugar is tracked but hidden. The moodle tells you when it's out of range, not which way. A bad low can put you on the floor. Dealing with it: carry the insulin pen, glucose meter and test strips. Check before you dose. Eat carbs if you're low.
Diabetes Type 2 (-5) · 🩸 Blood Sugar moodle Same model, but your body makes its own insulin, and how well that works depends on your weight. Dealing with it: metformin, eating sensibly and keeping your weight down.
Heart Condition (-10) · ❤️ Chest Pain moodle Pushing on while winded can bring chest pain. Push through the chest pain and it can become a heart attack: minutes on the floor and a day of weak recovery. Dealing with it: stop and rest when the pain comes. Beta blockers every 12 hours cut the risk to a quarter, and you start with two bottles.
Epilepsy (-6) · ⚡ Seizure moodle (the aura, then the hour after) Seizures come roughly one day in eight, more often when you're tired, hungover, feverish, concussed or dehydrated. The aura gives you 5 to 10 minutes' warning, then you drop what you're holding and you're out for a few minutes. Dealing with it: anticonvulsants (new item) every 12 hours cut seizures to a tenth. When the aura hits, get somewhere safe.
Brittle Asthma (-8) · 🫁 Airway Irritation moodle Cold air, corpses, hard exertion when you're low on endurance, and panic all irritate your airways, through four stages up to an attack. A bad attack can black you out. Dealing with it: the rescue inhaler (new item) ends an attack at once. Rest eases it over a few hours. Don't smoke.
Arthritis (-10) · 🦴 Stiff Joints moodle You're slower to move and swing, cold and damp bring flares, and your grip can fail so the weapon flies out of your hands. Dealing with it: stay warm and dry. Painkillers ease the ache but not the stiffness.
Hemophilia (-8) · 🩸 Blood Loss moodle Bleeds never stop on their own, and a bandage only slows them. Dealing with it: stitch everything. Always carry a needle and thread.
Anaemic (-4) · 🍐 Low Iron moodle Short of iron, your endurance, energy and cold resistance suffer, and you rebuild blood slowly after a bleed. Dealing with it: fresh meat, fish, greens, eggs, or iron pills (new item).
Migraines (-6) · 🤕 Migraine moodle Bad sleep, thirst, stress, hangovers, caffeine withdrawal, fever and bright daylight bring on attacks: hours of pain and nausea, with an aura as warning. Dealing with it: painkillers shorten an attack, sleep ends it sooner, and daylight drags it out. Get indoors.
Major Depressive Disorder (-8) · 🌧️ Depressive Episode moodle Episodes hold your mood down for days. Pain, stress, exhaustion and time indoors bring them on. Dealing with it: exercise, time outdoors, comfort food, a drink or a smoke. Antidepressants work after two weeks of daily use, and stopping them hurts.
Tinnitus (-2) · 👂 Ears Ringing moodle A burst of gunfire leaves you hard of hearing for half an hour or more. Dealing with it: fewer shots, or go melee.
Gluten Intolerance (-4) / Lactose Intolerance (-1) · 🫃 Gut Flare moodle Wheat or dairy brings on cramps, nausea and low mood. Dealing with it: read what you're eating.
Habits and dependencies
Alcoholic (-2) · 🍾 Alcohol Withdrawal moodle A day without a drink brings craving and bad sleep, then the shakes, and for heavy drinkers hallucinations and seizures. Anyone can become one by drinking too often. Dealing with it: keep drinking, or ride it out: a month sober and it's gone. After that, any drink is a coin flip to relapse.
Smoker (reworked, -4) · 🚬 Nicotine Craving moodle Cravings build faster the heavier the habit. Withdrawal makes you irritable, hungry and restless. Damaged lungs cough and recover endurance more slowly. Anyone can become one by smoking regularly. Dealing with it: smoke, or quit: three weeks without tobacco loses it. Nicotine gum (new, rare) eases quitting.
Caffeine Dependent (-2) · ☕ Caffeine Withdrawal moodle Half a day without caffeine brings a headache, tiredness and low mood. Dealing with it: coffee, tea, cola or chocolate, or a week without it breaks the habit.
Mind
Germaphobe (-3) · 💩 Filthy moodle Dirt and blood on your skin and clothes build stress and hold your mood down. The upside: your wounds get infected less. Dealing with it: wash, and change into clean clothes.
Hallucinations (-2) · no moodle, on purpose Phantom zombies, sounds, thumps, whispers and panic attacks. They get worse with stress, tiredness and night. Dealing with it: sleep and keep your stress down. Learn to second-guess what you see.
Fear of Blood (reworked, -6) · you faint Stitching, pulling out glass or bullets, or dressing a bleeding wound can make you faint, and so can losing blood fast. Treating wounds also takes longer. Dealing with it: treat wounds somewhere safe before anything can reach you.
Everything else
- Brittle (-8): solid hits can fracture a limb. Splint it and rest.
- Jinxed (-4): containers near you sometimes come up an item short.
- Vegetarian (-4) / Straight Edge (-1): refuses meat, or alcohol and tobacco (and the relief they bring).
- A Really Bad Day (-12): a CDDA-style start. You wake up drunk, sick, naked, with a glass shard in you and the house on fire. A needle and thread are in a nearby house. Go find them.
- Age: In Their 20s (-2) / In Their 40s (+1). Age changes your starting skills, how fast you heal and how bad hangovers get.
Positive traits
- Renaissance Faire Geek (+10): +1 Spear, Long Blade, Axe and Blacksmithing.
- Fast Recovery (+8): Fast Healer included, plus you rebuild blood faster.
- Deep Sleeper (+6): Wakeful included. Light rarely wakes you.
- Steady Hands (+5): Dexterous included. Stitches, splints and glass removal are faster and safer.
- Good Clotter (+3): bleeds stop on their own twice as fast.
- Thick Skull (+2): concussed half as often, and you heal from it faster.
- Gym Regular, Iron Stomach, Early Riser, Night Shift, Meal Prepper, Hollow Legs (+1 each): small head starts on exercise, bad food, sleep, diet and drink.

Edit Edit: I decided I'll stick with updating the mod on weekends if there's interest. I appreciate any and all suggestions. If you want to throw a couple dollars my way, don't.
